Job Summary
We are seeking to recruit a positive, caring and self-motivated individual to take on the role of Practice Nurse at Newport Health Centre.
Your main responsibilities may include:
1. Long Term conditions Management (For example, asthma, diabetes etc)
2. Wound care
3. Cytology
4. Venepuncture
5. Routine immunisations
6. Involvement in QOF achievement
7. Infection prevention control
As an autonomous practitioner, you are responsible for the care delivered, demonstrating critical thinking and skills in clinical decision-making in the management of patients. You will work collaboratively to meet the needs of patients, ensuring the highest standards of care, supporting the delivery of policy and procedures.

Newport Health Centre is based centrally on the Isle of Wight operating across two sites; situated at our Carisbrooke practice and our town centre practice, Dower House, Pyle Street.
Our practice supports over 24,000 patients on the Isle of Wight where we have a diverse, multi-disciplinary team which includes GP Partners, Advanced Nurse Practitioners, Paramedics, MSK Practitioners, Mental Health Practitioners, Practice Nurses, HCAs, Chronic Pain Coach, Care Coordinators, a Social Prescriber and many more vital roles to support our community. We are also an active part of the Central and West Primary Care Network (PCN).
Job responsibilities
Main Responsibilities
1. Take responsibility for own development, learning and performance including participating in clinical supervision and acting as a positive role model.
2. Promote and work to the Practices Vision and Values.
3. Assess, plan, develop, implement, provide and evaluate treatment programmes including, but not limited to:
1. Chronic Disease management. To work with the practice leads and where appropriate take a lead in developing and delivering specialist primary care services, such as long term disease management.
2. Health promotion and well-being plus individual plans for patients with/at risk of a long term condition.
3. Manage and treat long-term conditions, in line with national and local policies and practice needs.
4. Prioritise health problems, intervening appropriately, to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
5. Implement and participate in vaccination and immunisation programmes for both adults and children including patients travelling abroad.
6. Meet the needs of patients presenting for opportunistic wound care.
7. Diagnose and treat minor illnesses.
8. Recognise and work within own competence and professional code of conduct.
9. Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures.
10. Prioritise, organise and manage own and others workload in a manner that maintains and promotes quality.
11. Deliver care as an individual and team according to NSF, NICE guidelines and evidence-based care.
12. Implement and review the application of evidence-based practice in nursing.
13. Plan and manage nursing appointments across the Practice sites.
14. Evaluate patients response to health care provision and the effectiveness of care.
15. Work within policies regarding family violence, vulnerable children and adults, substance abuse and addictive behaviour, and refer as appropriate.
16. Participate in local implementation strategies that are aligned to the values and culture of general practice.
Person Specification
Qualifications
* Maths and English GCSE or equivalent experience.
* Registered nurse with NMC.
Skills
* Good interpersonal skills with the ability to communicate effectively with a diverse range of people.
* Effective time management (Planning & Organising).
* Ability to work as a team member and autonomously.
* Problem solving & analytical skills.
* Excellent attention to detail.
* Clinical skills e.g. cervical cytology, immunisation and vaccination, ear care, wound management, minor illness, contraception.
