This excellent GP surgery based over 2 sites in Lowestoft is looking to add to our clinical team. We are seeking an ANP or Nurse Practitioner who can work autonomously or part of a team.
Main duties of the job
On a daily bases you will be triaging and seeing patients face to face running your own clinics. You will deal with referrals, scripts, test results and help colleagues when necessary. Must be flexible, hardworking and willing to muck in when needed. Ideally the candidate will see all patients from young children to mature adults and have experience in dealing with all types of medical conditions. We also look after a local care home and provide twice weekly ward rounds.
About us
We have a very hardworking, friendly and very supportive team both clinically and administrative. We are a teaching and research practice. We are based over 2 sites and have 15300 patients between both sites. Our team consists of 3 GP Partners, 2 Salaried GPs, 3 NP/ACP, 5 Practice Nurses and 5 HCA's we have 2 Pharmacists and 3 Pharmacy technicians, also a large Reception/Admin team. We will offer a good rate of pay, 6 weeks holiday and pro rate bank holidays and NHS pension. We work with the Lowestoft PCN and have several PCN staff members who work with us and are part of our team.
Job responsibilities
The post holder is an experienced nurse who, actingwithin their professional boundaries, will provide care for the presentingpatient from initial history taking, clinical assessment, diagnosis, treatmentand evaluation of care. They will demonstrate safe, clinical decision-makingand expert care, including assessment and diagnostic skills, for patientswithin the general practice. The post holder will demonstrate critical thinkingin the clinical decision-making process. They will work collaboratively withthe general practice team to meet the needs of patients, supporting thedelivery of policy and procedures, and providing nurse leadership as required.In order to work at this level Nursing and Midwifery Council (NMC) requirementsfor advanced practice must be met and kept up to date.
Clinical practice
Assess, diagnose, plan, implement andevaluate treatment/interventions and care for patients presenting with anundifferentiated diagnosis
Clinically examine and assess patientneeds from a physiological and psychological perspective, and plan clinicalcare accordingly
Assess, diagnose, plan, implement andevaluate interventions/treatments for patients with complex needs
Proactively identify, diagnose andmanage treatment plans for patients at risk of developing a long-term condition(as appropriate)
Diagnose and manage both acute andchronic conditions, integrating both drug- and non-drug-based treatment methodsinto a management plan
Prescribe and review medication fortherapeutic effectiveness, appropriate to patient needs and in accordance withevidence-based practice and national and practice protocols, and within scopeof practice
Work with patients in order to supportcompliance with and adherence to prescribed treatments
Provide information and advice onprescribed or over-the-counter medication on medication regimens, side-effectsand interactions
Prioritise health problems andintervene appropriately to assist the patient in complex, urgent or emergency situations,including initiation of effective emergency care
Support patients to adopt healthpromotion strategies that promote healthy lifestyles, and apply principles ofself-care
Support and manage health needs ofwomen presenting for family planning, cervical cytology or sexual healthconsultation
Assess, identify and refer patientspresenting with mental health needs in accordance with the NSF for MentalHealth
Implement and participate in vaccinationand immunisation programmes for both adults and children
Advise, support and administervaccinations for patients travelling abroad, where appropriate
Promote and deliver evidence-based carefor patients presenting with aural conditions
Meet the needs of patients presentingfor opportunistic wound care
Undertake minor surgery as appropriateto competence
Understand how to identify and act upon safeguarding concerns(about children and vulnerable adults) in an appropriate manner (to Level 3East of England Guidance on Safeguarding Children)
