Job summary
Braintree Primary Care Network has an exciting opportunity for a hardworking, enthusiastic and innovative pharmacist to join us on a full or part time basis. The role will be located primarily at one or two of our member practices but the postholder will be required to travel between all of the GP practices within our PCN.
Main duties of the job
1. Clinically assessing & treating patients, using expert knowledge of medicines, for specific disease areas.
2. Providing specialist expertise in the use of medicines and medicine information ( side-effects, dosage).
3. Providing pharmaceutical medication review & health education advice to patients, by face-to-face, telephone or notes-based review, as agreed with the practice.
4. Providing leadership on person-centred medicines optimisation & quality improvement.
5. Contributing to public health campaigns, including flu vaccinations, childhood immunisation & adult immunisation programmes.
6. Acting as a key contact, for all medication related queries.
7. Supporting improvements in clinical care, through practice-based audits and implementing change.
8. Running clinics where medicines are the main interventions anticoagulant monitoring clinics.
9. Conducting care-planning, including mental health, admission avoidance schemes, & end-of-life patients.
10. Reviewing daily pathology results and act on results.
11. Undertaking minor ailments triage & directing patients, based on clinical need.
12. Capturing relevant patient health information at all consultations including contributing to the quality outcomes framework.
13. Monitoring patients with complex, long-term conditions such as hypertension, diabetes and COPD, ensuring that medication is optimised, titrated, monitored and complied with.
14. Providing clinical advice & expertise on medication.

Qualifications
Essential
22. Completion of a degree in pharmacy and registration with the General Pharmaceutical Council.
23. Post-qualification experience in pharmacy (hospital, primary care or community), or working towards obtaining this.
24. Completion of CPPE pathway.
25. Qualified independent prescriber, or working towards this.
26. Member of the Royal Pharmaceutical Society (RPS).

27. Awareness of the breadth of common acute and long-term conditions that are likely to be seen in a general medical practice.
28. Ability to demonstrate general pharmaceutical skills and knowledge in core areas.
29. Ability to plan, manage, monitor, advise and review general care plans for patients with long-term conditions;
30. Ability to demonstrate accountability for delivering professional expertise and direct service provision as an individuals.
31. Ability to recognise priorities when problem-solving and identify deviations from normal patterns, referring to seniors or GPs with confidence, when appropriate.
32. Ability to follow legal, ethical, professional and organisational policies, procedures and codes of conduct.
33. Ability to assist patients in decisions about prescribed medicines and supporting adherence as per NICE guidance.
34. Non-judgemental attitude and ability to be flexible and adaptable.
35. Self-motivated with excellent communication skills.
36. Understand the need for patient confidentiality.
