The Practice Nurse will provide high-quality, evidence-based nursing care within the primary care setting. They will focus on chronic disease management, childhood immunisations, wound care, and general treatment room duties. The role involves working collaboratively with the wider healthcare team to improve patient outcomes, reduce health inequalities, and support the practices commitment to delivering patient-centred care. Key Responsibilities Clinical Duties Chronic Disease Management:Provide assessments, reviews, and ongoing management of patients with conditions such as diabetes, asthma, COPD, and hypertension. Childhood Immunisations:Administer vaccinations following national guidelines and maintain accurate records. Wound Care & Dressings:Provide treatment and follow-up care for wounds, leg ulcers, and post-operative dressings. Health Promotion & Prevention:Offer smoking cessation support, weight management, and lifestyle advice to patients. Womens Health:Conduct cervical screening, contraceptive advice, and menopause support (if trained). Treatment Room Duties:Assist with ECGs, phlebotomy, ear syringing, injections, and minor illness assessments. Medication Management:Support long-term condition patients with medication adherence, reviews, and prescribing (if qualified). Team Collaboration & Professional Development Work closely with GPs, pharmacists, healthcare assistants, and other clinicians to provide holistic patient care. Engage in multidisciplinary team meetings to discuss complex cases and contribute to practice development. Participate in continuous professional development (CPD) and maintain up-to-date clinical skills and knowledge. Assist in audits, quality improvement projects, and policy updates within the practice. Administrative & Compliance Responsibilities Maintain accurate and confidential patient records using clinical systems (EMIS). Ensure compliance with NHS Wales and Public Health Wales protocols, NICE guidelines, and infection control standards. HIW inspections and ensure the practice meets regulatory requirements
