Job summary GP Connect PCN is looking to employ an Advanced Nurse Practitioners to support the our Primary Care Network team in the delivery of nursing services, working as part of the PCN multidisciplinary team, delivering care within their scope of practice to the entitled patient population. The successful candidate will be using advanced autonomous clinical skills and a broad and in-depth theoretical knowledge base in line with the latest RCN guidance on ANP competencies Main duties of the job The Advanced Nurse Practitioner will work within their professional boundaries; Providing care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their care. To undertake surgeries working in an autonomous role including the management of minor ailments, chronic disease, family planning, home visits to include both acute visits and the management of chronic medical problems in the house-bound To provide telephone based triage, assessments and consultations. Demonstrate safe, clinical decision-making and expert care for patients within the general practice maintaining clinical excellence. Work collaboratively with the multi-disciplinary general practice team to meet the needs of patients, supporting the delivery of policy and procedures. In order to work at this level, NMC requirements for advanced practice must be met.
