The Senior Primary Care Medicines Management Technician is an integral part of the Community Health and Wellbeing Team (CHWT) and supports the pharmacist to manage patient medication records to limit waste and resynchronise medications. Working on the advice of the CHWT Pharmacist, the post holder will attend weekly MDT meetings and undertake home visits to take drug histories and counsel patients on medications. The post holder will liaise with other Community Pharmacy Services to ensure patients are supported with their medication administration at home.
Main duties of the job
The post holder will work within a range of clearly defined occupational policies and:
- Ensure a safe, evidence-based and cost-effective medicines management service is provided by pharmacy technicians to areas within Cwm Taf Morgannwg University Health Board (CTMUHB).
- Support multi-disciplinary working and the development of Medicines Management Services.
- Develop, implement, and monitor standards for Medicines Management Services delivered by Pharmacy Technicians and Pharmacy Support Staff within CTMUHB.
- Support the delivery of the Cash Releasing Efficiency Savings (CRES) Schemes.
- Support the quality and safety requirements in relation to Medicines Management across the locality.
- Audit and monitor Primary Care Medicines Management Services provided by Pharmacy Technical Staff within CTMUHB.
- Act as a role model and lead the training and development for Pharmacy Technicians providing Primary Care Medicines Management Services.

Detailed job description and main responsibilities
The post holder will:
- Take forward the national campaigns by acting as a focus, driving force and facilitator to allow CTMUHB staff to develop and achieve the areas targeted at Primary Care:
- Improving Medicines Management.
- Achieving efficiency.
- Reducing harm and variation.
- Supporting the Primary Care Teams to improve the experience and outcomes involving medicines for individual patients by:
- Addressing unmet pharmaceutical needs.
- Helping patients get the best from their medicines.
- Improving service efficiency and reducing waste.
- Improving access to Medicines Management Services.
- Lead and co-ordinate activities on agreed areas within the prescribing team and CTMUHB, relating specifically to the Medicines Management (MM) and patient safety.
Managerial responsibilities include:
- Development and management of medicines management services by Pharmacy Technicians.
- Support Clinical Pharmacists and Nursing staff providing Medicines Management Services to designated clinical area(s) in accordance with Professional Standards, Departmental and Health Board Policies and Procedures.
- Work closely with Specialist Pharmacists and other Healthcare Professionals in developing Primary Care Pharmacy Services.
- Write and develop protocols, standard operating procedures and service training packages for Primary Care Pharmacy Technical Staff.
- Review and update all Primary Care Medicines Management Technician procedures, and monitor compliance.
