Dimensions and Context of Role 7.1 The District Nurse Team Manager is responsible for the operational management of the locality-based District Nurse Team. 7.2 The role entails 24/7 responsibility, 365 days a year, for the District Nursing Team, supported by Band 6 nurses. 7.3 The manager provides leadership in service development and redesign, profiling local health needs within the locality, and identifying or addressing resource needs. 7.4 Active involvement in the strategic planning of health and community-based services, with responsibilities including deputizing for and supporting the Operational Lead / Directorate Manager and Professional Leads. 7.5 The role includes providing clinical leadership and fulfilling clinical duties within the team. 9. Key Tasks and Responsibilities Clinical Responsibilities: 9.1Ensure the privacy, dignity, and human rights of all patients and carers are respected. 9.2 Support the team in managing complex or urgent situations, such as NHS continuing healthcare and safeguarding concerns, in collaboration with patients and carers. 9.3 Assist caseload holders in managing and coordinating care packages. 9.4 Oversee the quality assurance of continuing NHS healthcare assessments within the team and across the locality. 9.5 Identify and challenge professional and organizational boundaries, developing and implementing improvements to ensure high-quality, patient-centred care. 9.6 Work with integrated multidisciplinary teams across health and social care to develop, implement, review, and evaluate care programs for patients and their carers. 9.7 Ensure care provided and services delivered align with national, local, and organisational targets. 9.8 Engage in health promotion activities for all age groups and contribute to planned health promotion initiatives. 9.9 Maintain accurate, confidential, up-to-date, and legible records of care provided, adhering to organisational policies, NMC guidelines, and government directives. 9.10 Oversee the quality of patient records through audits or peer reviews, and hold regular discussions with team members. 9.11 Comply with all infection control policies and guidelines, attend relevant updates, and report any concerns to the immediate line manager, escalating issues as necessary. 9.12 Monitor and maintain the health, safety, and security of yourself and others. Management and Leadership Responsibilities: 10.1 Serve as the operational manager for a locality-based District Nursing Team. 10.2 Set workload priorities for yourself and the team, balancing patient care, management responsibilities, and staff training needs within resource limitations. 10.3 Report, record, and manage all incidents and near misses related to health, safety, security, fire, physical violence, aggression, and verbal abuse. 10.4 Maintain a visible and accessible presence for both patients and staff within the organization. 10.5 Provide professional clinical support to the nursing team, advising on practice and identifying training needs, while encouraging practitioners to develop skills in critical thinking, reflective practice, and research-based practice. 10.6 Ensure regular, structured team meetings are held and documented. 10.7 Lead and participate in clinical audits and research projects aligned with the organisation's audit strategy. 10.8 Act as a role model, demonstrating high standards of practice and professional conduct, addressing issues of poor practice with senior line management and HR. 10.9 Coordinate team responsibilities to ensure equity of service provision and workload within the District Nursing Team. 10.10 Monitor and control the team's budget, liaising with the Management Accountant and Locality Manager to manage expenditure effectively. 10.11 Ensure robust processes are in place for the maintenance, monitoring, and authorization of equipment and supplies for the team. 10.12 Oversee duty rotas, ensuring effective use of resources, adherence to the roster policy, and rigorous control of bank and agency usage. Manage staff sickness and other leave while maintaining safe staffing levels. 10.13 Monitor staff performance and manage sickness. 10.14 Foster an environment conducive to team building, working, and development. 10.15 Lead and support the team through change processes, proactively addressing problems. 10.16 Provide and promote clinical and line management supervision for all team members in accordance with policies. 10.17 Ensure the appraisal process is conducted at least yearly, as per organisational policy. Undertake appraisals and maintain line management responsibility throughout the team. 10.18 Represent the District Nursing Service at a strategic level. 10.19 Work with the team and other matrons to review staffing establishments, skill mix, and workforce plans, ensuring that staffing meets current and projected clinical demands. 10.20 Lead recruitment and selection processes, ensuring adherence to organisational and employment procedures. 10.21 Handle complaints and conflicts within the team in accordance with organisational policies. 10.22 Oversee the ongoing monitoring and updating of the Electronic Staff Records. 10.23 Serve as a direct link between the team and the Operational lead / Directorate Manager, liaising with on any areas of concern. 10.24 Ensure that protective management time is included within the weekly work plan. 10.25Plan, facilitate, and monitor the professional leadership and managerial skills of caseload holders to ensure they perform at the required level. 10.26 Maintain a flexible approach to the working day to meet the needs of patients and the service. 10.27 Be aware of and support colleagues facing work pressures, ensuring they are informed about organizational support services available to them. 10.28 Act as the team lead for the risk register and escalate risks across the team as necessary. Communication Responsibilities: 11.1 Demonstrate a clear understanding of the District Nursing Service, Primary Care, organisational, and government objectives, and effectively communicate these to others. 11.2 Communicate effectively at all levels within the organization, with a variety of health professionals, users, and carers, to achieve the best outcomes for patients within the primary care setting. 11.3 Ensure that staff complete and submit all necessary forms, documentation, and IT data in a timely manner as required by the manager/organisation. 11.4 Act as a patient advocate when needed to ensure individual needs, preferences, and choices are understood and communicated to all involved in care delivery. 11.5 Use equipment provided by the organisation for work purposes, abiding by the organisation's policies. Education and Training Responsibilities: 12.1 Facilitate and ensure equitable training for all team members, supporting infrastructure for induction, preceptorship, professional development, and student placements. Regularly review the career development of staff within the team through line management and appraisal processes. 12.2 Take responsibility for your own professional development and personal knowledge. 12.3 Monitor your own performance against agreed objectives through the appraisal process, personal development plans, and NMC regulations. 12.4 Participate in the supervision, teaching, and provision of community experience for students and other personnel as appropriate. 12.5 Promote lifelong learning and staff development by fostering a supportive learning environment.
