About the Role
As an NDIS Support Coordinator for Mark & Sylvie's Home Care, you will guide a team of caregivers to deliver the highest level of disability services. Your leadership will ensure that each client receives personalised, high-quality support. This is a part-time role, working 20-25 hours per week.
Key Responsibilities:
1. Lead and Inspire: Manage and motivate a team of compassionate caregivers, setting a standard of excellence.
2. Personalise Care Plans: Develop and implement tailored care plans that honour each client’s unique needs.
3. Ensure Compliance and Quality: Oversee adherence to healthcare regulations and foster best practices.
4. Client-Focused Collaboration: Work closely with healthcare professionals, clients, and families to tailor services to individual needs.
5. Resource Management: Optimise budgets, resources, and client service agreements to ensure the highest quality of care.
6. Champion Improvement: Lead quality initiatives to enhance our innovative home care services.
What We’re Looking For:
1. Experienced NDIS Support Coordinator or Team Leader: Holding a Certificate IV in Disability/Ageing Support or Diploma in Community Services, you will be familiar with coordinating services under the NDIS. Some familiarity with home care packages would also be advantageous (though not essential), since you will be required to assist this team along the way.
2. Leadership and Organisation: A strong ability to lead and inspire teams.
3. Strong Communication Skills: Exceptional interpersonal skills to foster positive relationships with clients, families, and team members.
4. Knowledge of Healthcare Standards: Solid understanding of NDIS regulations and best practices.
5. Tech-Savvy: Experience with electronic health records and care management software is a plus.
