Job Summary
The Physiotherapy Department is a dynamic, friendly and cohesive department and we are looking to recruit a motivated and ambitious individual to work within our Hospital Outreach Support Team (HOST).
We are looking to recruit an experienced band 5 or current band 6 physiotherapist for a full-time position. The successful candidate will demonstrate excellent multidisciplinary team working, adaptable communication for complex patients and be innovative in their approach to support further development of this busy, dynamic department.
You must have experience within the NHS, preferably within the acute setting and also be a car driver/owner. There is a requirement for flexible working as the service currently covers a 7 day working week, including bank holidays, using a rostered system.
Main Duties of the Job
HOST is a highly regarded and experienced team supporting discharges and helping prevent readmission of complex patients from all specialities and demographics. By reviewing patients in their own homes, HOST provides short-term patient-centred rehabilitation, adaptive aids and advice to help patients reach their functional goals. In addition, HOST supports patients with management of orthopaedic devices including cervical collars, upper and lower limb braces.

Job Responsibilities
To provide physiotherapy to relevant patients within HOST, in a range of settings including inpatients, outpatients and patient homes.
To assess and treat patients in HOST. To support the team in the organisation and provision of an efficient, high quality and co-ordinated physiotherapy service for patients in each area. To undertake a defined clinical caseload. To ensure that patient and service standards and priorities are met. To provide clinical supervision to assigned junior physiotherapists, technical instructors, physiotherapy assistants and students. A rotation with inpatient services may be considered.

Qualifications
Essential:
* HCPC registration
* BSc Physiotherapy
Desirable:
* Member of CSP
* Member of Specific interest group
* Post graduate qualifications in related area i.e. frailty
Knowledge
Essential:
* Previous Acute NHS experience at band 5 level
* Achieved core skills in all core Junior rotations, including elderly care and orthopaedics
* Evidence to support knowledge base in specialist area
* Previous acute NHS experience
Desirable:
* Previous Senior Experience in Specialty Area
