Job Summary
Vertis Health is looking for a proactive and passionate Health & Wellbeing Practitioner to support our community-facing Your Health programme across Worcestershire. This is a part-time role at 11 hours per week.
This role offers the opportunity to deliver vital public health services directly to local communities, including NHS Health Checks, point-of-care testing, immunisations, screening, lifestyle advice, and phlebotomy. The successful candidate will drive the Your Health mobile treatment van to local venues, engage face-to-face with diverse groups, and work collaboratively with GPs, social prescribers, and other health partners.

Main duties of the job
* Deliver NHS Health Checks and associated screening services.
* Provide lifestyle advice and signposting relating to physical activity, healthy eating, alcohol, smoking, and mental wellbeing.
* Undertake immunisations, phlebotomy, pre-screening for refugees/asylum seekers, and swab testing during disease outbreaks.
* Drive and set up the mobile van at community locations.
* Manage stock, vaccines, and reporting systems (EMIS).
* Collaborate with GP practices and local services to support patient needs and reduce health inequalities.
* Promote the Your Health programme through outreach and social media engagement.

Job Responsibilities
This is a unique and rewarding opportunity to work at the heart of communities, reaching people where they live, work, and socialise to deliver life-changing healthcare services. As a Community Health and Wellbeing Practitioner with the Your Health mobile service, you will work across multiple sites including community centres, pop up clinics, and the Your Health van to bring essential care to under-served and vulnerable populations.
Every day will be different. You could be delivering NHS Health Checks in the morning, giving lifestyle advice in a city centre clinic at lunchtime, and supporting an immunisation or swabbing event by the afternoon. You will play a key part in delivering prevention, early intervention, and rapid response support that tackles real health inequalities and improves lives.
Clinical and preventative health services
* Deliver NHS Health Checks in community locations, using point of care testing equipment
* Provide screening, lifestyle advice, immunisations, and education on key health behaviours
* Undertake phlebotomy and pre-screening for new patients including refugees and asylum seekers
* Respond to public health incidents by supporting with swabbing and immunisations as required
Patient support and signposting
* Offer practical advice and support on healthy eating, physical activity, mental wellbeing, smoking and alcohol use
* Signpost to other services such as GP practices, Stop Smoking services and Social Prescribers
* Register patients with a GP where appropriate and provide onward referral support
Community outreach and engagement
* Drive and set up the mobile treatment van to deliver pop up services across the county
* Build strong working relationships with community groups, partners and local organisations
* Support service promotion through events and social media in line with guidelines
Partnership and team working
* Work closely with the programme team including clinical leads and operational managers
* Liaise with GPs, pharmacies and other healthcare teams to coordinate care and referrals
* Share patient information in line with consent and data sharing protocols
Monitoring, reporting and improvement
* Use clinical systems including EMIS to record consultations, assessments and follow up
* Support with performance reporting and quality improvement by gathering patient feedback
* Help review stock levels, equipment and supplies for the van and base site
* Contribute to service development through team meetings and business planning
Person Specification
Skills
* Strong communication and interpersonal skills
* Ability to manage caseloads and prioritise tasks
* Understanding of NEWS2 and appropriate escalation
* Data input and attention to detail
* Awareness of safeguarding and NHS Health Checks
* Familiarity with GDPR and data protection

Experience
* Previous experience in a healthcare/community setting
* Health promotion or patient engagement experience
Qualifications
* NVQ Level 3 in Health or Social Care or equivalent experience
* Full UK driving licence
