Job summary You will be one of two B7 Patient Educator & Health Coaches on the London Spinal Cord Injury Centre.The role requires you to oversee and co-ordinate the patient education programme, meeting the educational and rehab needs of patients on LSCIC. This involves the training and promotion of health coaching within the LSCIC ward staff and the wider MDT to enable them to promote patient centred care & patient self-management. You will work clinically to facilitate education and promote best practice within the London Spinal Cord Injury Centre. You will have excellent communication skills, and promote Health Coaching and patient education via a variety of sources.You will work alongside the other Patient Educator & health coach, Clinical Nurse managers, Ward matron and Clinical Educators. Main duties of the job You will be responsible for promoting the Health Coaching role within the LSCIC ward team and wider MDT promoting both patient centred care and patient self-management. To be successful in this post you be required to work autonomously and demonstrate excellent communication skills. You will be required to work clinically to facilitate education and promote best practice within the London SpinalCord Injury Centre. The Centre is supported by a dedicated Educational team including Clinical Nurse Educators, Clinical Nurse Managers and a Ward Matron. About us This centre is a specialist commissioned 30 bedded Regional Rehabilitation Centre and one of only 8 centres in England. This service facilitates rehabilitation of individuals who have been recently paralysed as a result of traumatic or non-traumatic events. The service also re-admits individuals for tissue viability care, urology input and secondary episodes of rehabilitation.The overarching philosophy of the specialist centre is that rehabilitation is a journey that takes years to achieve and is best delivered through lifelong interventions and learning. The MDT aim to give the patients the initial skills to return to their lives as soon as possible. The patient journey is further supported by re-admission opportunity's, outpatient services, specialist community liaison nursing, all to maintain good health and ongoing skill acquisition.
