Our practice serves a diverse patient population of over 9, 300 people. Our growing multi-disciplinary team, which includes Doctors, Advanced Nurse Practitioner, highly skilled nurses, prescribing team as part of the care navigation and administration team who are committed to delivering high-quality, patient-centred health care while embracing innovation and collaborative working. We also undertake vocational training of new General Practitioners and practice nurses as part of the GP training programme and Wales Academy for learning in Primary Care. Main duties of the job As part of our multi-disciplinary team (MDT), you will: Hold face-to-face and telephone medication reviews, optimising treatment for long-term conditions and the management clinics which might include DOAC clinics and chronic disease management. - Provide expert medicines advice to GPs and other healthcare professionals - Lead Quality Improvement projects and clinical audits and drive the prescribing team forward with innovation and high-quality patient care and practice. - Ensure safe and cost-effective prescribing, helping manage medicines post-hospital discharge.
