Job overview
We are looking for a highly motivated, enthusiastic and experienced nurse to join our Community Heart Failure CNS team in Tower Hamlets. This post is part time to cover Thursdays and Fridays.
The Heart Failure clinical nurse specialist is a role for an independent practitioner working within the multi-disciplinary cardiology team. The post holder will work closely with the whole multidisciplinary team to coordinate the clinical care of heart failure patients within the borough.
Responsibilities include education of patients and colleagues, coordination of patient care between GPs, community, and hospital, including admission avoidance, systematic data collection, clinical audit, and MDT meeting attendance. The post holder will develop specialised skills and knowledge in the field and perform nurse-led clinics and independent home visits.
Main duties of the job
* Work with health professionals to improve awareness and diagnosis of heart failure and ensure good practice systems are in place.
* Maintain standards of care according to clinical guidelines (NSF for CHD and NICE guidelines) and protocols.
* Collaborate with MDT to provide comprehensive care for heart failure patients.
* Provide access to care and support through exercise and education programs and focus groups.
* Improve patient care standards through evidence-based research and participate in the National Heart Failure audit.
* Coordinate palliative care support for end-stage heart failure patients and ensure seamless care along the pathway.
* Conduct nurse-led clinics and home visits to manage symptoms and medication optimization.
* Identify working groups to develop guidelines and policies.
