Job Summary:
This position serves as the educator for the clinical and non-clinical homecare staff and is responsible for initial orientation, ongoing education, and support for clinical and non-clinical staff. Responsible for developing and implementing a comprehensive clinical informatics education program. Utilizes review of research and current evidence–based practice to recommend changes in both practice and the clinical information systems. Educates staff via didactic training, joint visit observation, and documentation review as well as in-services. Coordinates and collaborates with multiple disciplines and clinicians to ensure EMR meets all regulatory requirements. Provides problem solving and troubleshooting for staff and managers. Liaisons with EMR vendor and CNE IS. Prepares/presents and communicates key messages and homecare educational programs.
In addition to serving as educator, this individual will analyze data to evaluate performance in quality improvement, develop and implement activities to improve overall patient experience, claims based, and quality measures as measured by nationally standardized benchmarks and definitions. This candidate will be responsible for various duties, including medical record auditing, associated with the continuous monitoring, maintenance, and improvement of quality throughout the organization. Lead and participate in special projects as directed and perform other duties as assigned. Maintains and manages the job-related quality and regulatory assignments, including preparing and distributing reports, submitting quality data, and assisting with the coordination of committee and regulatory survey logistics. Manages processes to ensure accuracy and completeness of work. Knowledgeable of department policies and quality procedures appropriate to the position. Works collaboratively with the team to complete required area of responsibilities and special assignments.
Duties and Responsibilities:
1. Orientation:
o Coordinates homecare orientation for all clinical and office staff (as applicable) and evaluates effectiveness.
o Coordinates and arranges patient assignments during orientation.
o Assesses initial competency as relates to practice area, patient clinical needs, and regulatory requirements for clinical documentation.
o Initiates and completes clinical skills assessment and orientation checklist.
o Collaborates with manager to complete the probationary evaluation for all clinical and non-clinical staff.
o Evaluates the effectiveness of orientation in relationship to specific measures.
o Conducts joint visits with staff to accomplish orientation and assess competency.
o Conducts competency assessments as needed.
2. Ongoing Education:
o Proactively identifies staff learning needs as identified by report review and direct observation during office and home visits.
o Prepares, presents, and evaluates the effectiveness of in-services and educational sessions.
o Develops and manages a master education calendar. Presents proposed education and summary of previous events at appropriate committee.
o Develops and coordinates agency-wide annual competency fairs.
o Assists in securing resources for specialty education needs.
o Participates on agency, hospital and external committees.
o Identifies and assists in providing opportunities to develop/expand clinical expertise of staff and maintain timely competencies.
o Acts as a resource for clinical staff.
o Collaborates with the leadership team to provide sound and relevant educational experiences for staff.
o Provides continuity of care by assisting with linking patient needs with the appropriately skilled clinician.
o Educates clinicians on case management practices to ensure appropriate utilization, consistency in visits and high-quality outcomes.
o Participates in and reports departmental quality indicators to the VNA Quality Improvement Committee.
o Evaluates the effectiveness of educational sessions. Reports results and uses data to make changes.
o Collaborates in providing education related to TJC, DOH, regulatory requirements.
o Supports and provides education on new clinical initiatives (ex cardiac care, wound, etc).
o Works with managers to develop, implement, and evaluate PIP for staff who need additional assistance.
o Develops and reviews policies/procedures as required.
o Involves managers and other staff as necessary regarding the aspects of planning and implementing software changes.
o Integrates all training on computer systems and documentation within the context of staff responsibilities.
o Provides orientation and ongoing training to all clinical and non-clinical staff regarding EMR and other platforms.
o Provides training/support for staff as it relates to technology.
o Creates customized process/screens for use by specialties to meet documentation requirements.
o Prepares, develops, and presents in-service and educational sessions.
o Healthstream (CBL) expert for the agency including running reports, creating classes and courses. Coordinating annual mandatories with HealthStream Manager for the system.
o Identifies and evaluates instructional materials in all types of media and formats.
o CPR coordinator for VNA.
3. Competency Assessment:
o Manages the competency assessment process, including the initial competency assessment conducted during the probationary period, as new skills are required, or quality issues are identified.
o Ensures competency assessment is documented and data accessible.
o Assists in development and coordination of annual competency fairs.
o Assists homecare management to manage clinical placement requests and orientation of students as needed.
4. Quality:
o Provide Support and Guidance: Act as a resource for clinical staff related to OASIS, coding, and PDGM guidelines, HHVBP, offering guidance and support as needed.
o Conduct Quality Reviews: Regularly review patient records and documentation to ensure accuracy and compliance with home care regulations and work with homecare managers to address any discrepancies or non-compliance issues promptly.
o OASIS review: Ensuring the accuracy, appropriateness, and completeness of OASIS assessments. Data collected is consistent with patient conditions and regulatory requirements, review charts and identify areas that need correction. COPs: Assist with chart review process and collection of data.
o Analyze data collected through dashboards to identify trends, gaps, and areas for improvement. Generate comprehensive reports based on dashboard data, providing actionable insights for management and staff.
o Regularly review dashboard metrics to ensure compliance with industry standards and regulatory requirements. Track performance against established quality targets and benchmark against best practices.
o Collaborate with cross-functional teams to implement quality improvement initiatives based on dashboard findings. Conduct training and workshops on using dashboards effectively for quality management.
o Collaborate and coordinate with organization-wide QI staff to develop/review all processes involved in monitoring, delivery of care, regulatory requirements which impact agency care processes.
o Investigate incidents and adverse events to determine their root causes and develop strategies to prevent recurrence.
o Subject matter expert in clinical nursing practice, quality, and management of complex conditions.
o Supports and facilitates both quality and clinical team relationships including care, risk, quality, and compliance work streams.
o Responsible for assisting with the development of programs and initiatives for quality monitoring and performance improvement throughout the organization.
o Assists the staff with the development of best practices and guides the implementation of evidence-based practice where applicable.
o Assists with the development of guidelines, criteria, and clinical indicators, which are applied to measure quality of care.
o Compiles data and information for departmental projects and presentations.
o Prepares documents and reports that are accurate, complete, and within required time frames.
o Maintains all department documents and files so they are complete, accurate, and retrievable.
o Assures policy/procedure binders and computer files/folders are up-to-date and accurate. Archives/scans old policies as they become outdated.
o Coordinates and arranges meetings, prepares agendas, reserves, and prepares meeting facilities, records, & transcribes meeting minutes in an accurate, timely, confidential manner.
o Conducts and coordinates projects as assigned; prepares and presents findings in an organized fashion, ensures timely completion of assignments.
o Creates charts, graphs, and displays data appropriately.
o Patient/Family Experience - Prepares and distributes individual team and staff quality outcome and HHCAHPS reports.
o Regulatory - Serves as support for TJC, State surveys, quality committee and other initiatives as needed.
o Accesses and utilizes the Joint Commission site, SHP, Shared Point, EMR, and keeps current on processes or IS upgrades to systems workload.
Requirements:
* Minimum Associate Degree in Nursing. Must be enrolled and/or working toward a Bachelor degree in Nursing with a commitment to complete BSN within 3 years.
* Current RN licensure in State of Rhode Island required. Must possess a valid driver’s license and current automobile liability insurance. Must be BLS CPR certified.
* Must have a minimum of three (5) years progressive clinical nursing experience required. Expertise in homecare regulations, computer literate, excellent communication skills (able to coach and teach). Attention to detail, excellent follow through. Previous experience in a hospice/home health setting preferred.
* Ability to read and interpret documents such as medical records, medical reports, etc. Ability to write routine reports and correspondence. Must be able to speak, read, and write English with excellent oral and written communication skills required.
* Ability to define problems, collect and analyze data and draw valid conclusions. Ability to interpret an extensive variety of technical instructions furnished in written, oral, diagram or schedule form. Must be able to deal with several abstract and concrete variables.
