Responsibilities:
* Conduct comprehensive assessments of service users to determine their needs and eligibility for services.
* Develop, implement, and review care plans in collaboration with service users and their families.
* Coordinate and manage safe and timely discharges from hospital settings.
* Work collaboratively with other professionals such as doctors, nurses, and therapists to ensure a holistic approach to care.
* Advocate for the rights and needs of service users, ensuring they have access to the resources and support they require.
What we are looking for in a Social Worker:
* Social Work England registration
* Eligible to work in the UK
* Hold a full UK licence
* Post-qualified experience
* Degree level or equivalent in Social Work
