Job summary We are looking for a ANP with a special interest in the older age patients. The ANP will work in the care homes looked after by the South East Oxford Health Alliance. These consist of 6 nursing and care homes in the south east of Oxford City. You will work closely with GPs and will have a named GP clinical supervisor Main duties of the job The ANP will provide clinical services to the patients living in the 6 care homes including chronic conditions reviews, medication reviews and acute issues. They will liaise with external agencies, care home teams and the GP Practice where the patient is registered. About us South East Oxford Health Alliance is a growing PCN providing services to 21,000 patients. It consists of directly recruited paramedics, social prescribers, learning disabilities coordinator, pharmacists and care home ANP coordinator. As well as a team of administrative support staff. Each care home is looked after by a named GP from the responsible surgery who is the ANP's point of contact. Chronic condition reviews (e.g. COPD, heart failure) and coding of these on the medical system Clerking new patients and updating medical record medication review and liaising with GP to review meds and deprescribe where appropriate Clinical examination of patients, and procedures as needed- e.g. bloods, catheter care Handover to care home after debrief with GP as to actions taken, and anything they need to do Structure medication review and liaising with pharmacists and GPs regarding structured medication reviews queries Liaising with operations officer regarding ensuring correct coding of activity, care plans etc. Proactive care planning and end of life care ensuring anticipatory meds have been prescribed and collected, ensuring DNAR forms and proactive care plans are completed etc. Liaising with care home support service and other support organisations (eg.
