Active Care Group’s Care in the Home division continues to go from strength to strength ensuring we provide a safe, reliable and outcome-based care to all clients by our exceptional support workers, clinicians, and management team.

We have a wonderful opportunity to join our friendly team of Experienced Care Co-ordinators supporting our Care in the Home division. You will be providing a premium customer service, co-ordinating and scheduling our care packages to enable our clients to receive continuity of care in their own homes.
Our care in the home services support children, young people and adults with a wide range of conditions, including: brain injury, spinal cord injury, neurological conditions and neurodegenerative diseases, respiratory conditions and gastro-intestinal conditions, including percutaneous endoscopic gastrostomy (PEG) feeding.
What you'll be working:
1. 9am until 5:30pm
2. Part of the on-call rota, Monday to Thursday or Friday to Monday
What you'll be doing:
3. Inbound and outbound calls to both clients and workers to check welfare and the cover of shifts
4. Keeping the Team Leader updated with anything business critical that happens within the team daily
5. Ensure that compliance with company policies and procedures is maintained in all locations and that each service achieves exacting standards of regulatory compliance – specifically continuity of care, commissioned vs delivered, safeguarding and complaints
6. Ability to provide and receive complex, sensitive, and contentious information and communicate effectively with staff at all levels, including those who use our services and members of the public
7. Identifying and collaborating on innovative ideas, demonstrating vision, and supporting creative, flexible, and responsive workplace
8. Always develop and deliver best practice and ensure that workloads are met in a timely manner
9. Competently represent the company and maintain/develop positive relationships with internal and external stakeholders
What you'll have:
10. Experience as a Care Co-ordinator
11. Previous experience of using booking and phone systems
12. Ability of dealing with conflict in a claim and supportive manner
13. Strong work ethic is essential, as is a dynamic and flexible approach to the role
14. Excellent customer service skills
15. Be able to work as a team, alone and think outside of the box
16. Excellent communication and interpersonal skills
17. Willingness to learn and develop
18. Successful candidates will be required to undergo an Enhanced DBS
