Job summary
Broxbourne Alliance Primary Care Network has an exciting opportunity for a hardworking, enthusiastic & innovative clinical pharmacist to join our PCN, on a full time basis for hours per week. You will be required to travel between all of the 4 GP practices within our PCN.
Main duties of the job
1. Clinically assessing & treating patients, using expert knowledge of medicines, for specific disease areas.
2. Providing specialist expertise in the use of medicines and medicine information.
3. Providing pharmaceutical medication review & health education advice to patients, by face-to-face, telephone or notes-based review.
4. Providing leadership on person-centred medicines optimisation & quality improvement.
5. Supporting practice dispensary.
6. Acting as a key contact, for all medication related queries.
7. Running clinics where medicines are the main interventions anticoagulant monitoring clinics;
8. Conducting care-planning, including mental health, admission avoidance schemes, and end-of-life patients.
9. Undertaking minor ailments triage and directing patients, based on clinical need.
10. Capturing relevant patient health information at all consultations including contributing to the quality outcomes framework.
11. Providing clinical advice and expertise on medication.
12. Managing the repeat prescribing reauthorisation process, by reviewing requests for repeat prescriptions and medicines reaching review dates.
13. Reviewing prescribing against local and national targets, and engaging with the wider prescribing team to rectify any issues.
14. Carrying out structured medicines-optimisation reviews in care homes.
15. Contributing to the reduction of medicine wastage within practices across the PCN.

23. Ability to demonstrate general pharmaceutical skills and knowledge in core areas.
24. Ability to plan, manage, monitor, advise and review general care plans for patients with long-term conditions.
25. Ability to demonstrate accountability for delivering professional expertise and direct service provision as an individuals.
26. Ability to recognise priorities when problem-solving and identify deviations from normal patterns, referring to seniors or GPs with confidence, when appropriate.
27. Ability to follow legal, ethical, professional and organisational policies, procedures and codes of conduct.
28. Ability to assist patients in decisions about prescribed medicines and supporting adherence as per NICE guidance.

Qualifications
Essential
32. Completion of a degree in pharmacy and registration with the General Pharmaceutical Council.
33. Post-qualification experience in pharmacy (hospital, primary care or community), or working towards obtaining this.
34. Qualified independent prescriber, or working towards this.
35. Member of the Royal Pharmaceutical Society (RPS).
