PRACTICE NURSE - Salary dependent on experience
We are seeking a dedicated and compassionate Practice Nurse to join our existing nursing team in providing high-quality medical care to our 22,000 patients. The ideal candidate will have a passion for patient care and some experience working in a primary care environment, with qualifications and experience in some or all of the duties. A qualification or interest in diabetes would be beneficial.
Main purpose of Post: To work alongside medical personnel in the care of patients within the medical centre, including treatments, preventative care, screening, and patient education. To provide a high standard of patient care, making the best use of clinical skills within the practice.
To liaise with the nursing team to provide a streamlined service within the medical centre. To provide support to junior members of the team, including Health Care Support Workers. Good communication and relationships must be established and maintained with patients, colleagues, and employers at all times. Follow guidance and practice policies at all times.
Treatment Room Duties: To undertake the following procedures adhering to practice policy and guidance. Training will be provided in this area if required and deemed necessary:
1. Patient assessment
2. Ear Irrigation
3. ECGs
4. D-Dimer testing
5. Doppler
6. Blood pressure management including titration of medications within practice policy
7. Wound care
8. Venepuncture
9. Injections including Zoladex, Flu vaccines, B12, Pneumonia, covid-19
10. Contraceptive Advice including depot injection
11. Travel Vaccinations and assessment
12. Child Immunisations and monitoring of immunisation schedules
13. Monitoring and ordering of childhood vaccinations
14. Cervical cytology
15. Spirometry and interpretation
16. Undertaking policy reviews and amendments where required
17. Swabs labelling and sending to the lab, actioning these results where appropriate
Assisting, if/when required, the GP in minor surgery in the absence of HCSW.
Health Promotion and Patient Education Responsibilities: To manage chronic disease as outlined within QOF requirements in accordance with practice policy and NICE guidance:
1. Diabetes patient education
2. Asthma patient education
3. COPD patient education
4. CHD patient education
5. Health promotion/education
6. Family planning advice
7. Pre-conceptual advice
Administration and Professional Responsibilities: Ensure accurate and legible notes of all consultations and treatments are entered in patients' electronic records according to practice policy. Ensure accurate completion of all necessary documentation associated with patient health care within the practice. Ensure collection and maintenance of statistical information required for regular ad hoc reports and audit. Develop practice protocols as required following senior clinician guidance and NICE recommendations, including the development of computerized templates. Participate in the review of significant and near-miss events. With nursing colleagues, undertake risk assessment where applicable.
Attend and participate in both practice and nursing meetings as required. To undertake research studies independently or in conjunction with other health care professionals with the support and guidance from clinical nurse lead and GPs, with the objective to improve or re-evaluate nursing care and services.
Training and Professional Development: Training and development will be monitored by yearly appraisal and the formulation of a personal learning plan in conjunction with the practice learning plan. Professional development will be encouraged and supported by the practice.
It is the individual's responsibility to keep their CPD up to date and ensure they are registered with the NMC and revalidation. It is the responsibility of the nurse to ensure they develop and maintain a personal learning plan. Participate in the education and training of students of all disciplines and the introduction of all members of the practice staff where appropriate. If it is necessary to expand the role to include additional responsibilities, appropriate training will be given.
To provide cover for colleagues in cases of absence, including the late night nurse role and cover for some extended hours clinics where appropriate. To provide additional duties as requested by senior nurse, management, or partners.
Liaison: Along with the nursing team, there is a need to work collaboratively with reception and administration staff to ensure the smooth running of the practice, reporting any problems encountered to the relevant person/persons. There will also be a need to establish and maintain good liaison with other surgeries, agencies, and secondary care.
Ensure that the practice manager is informed of any matters arising or problems/potential problems. The partners reserve the right to redistribute duties and functions amongst members of staff from time to time, and requests for such things shall not be unreasonably refused. You must at all times be aware of and concerned with our health and safety at work and fire safety policies.
