Main duties of the job
Your Role: As a primary care ACP, you'll play a key role in managing minor illnesses, common GP surgery presentations, and long-term conditions. You'll work both independently and collaboratively, ensuring our patients receive the highest standard of care.
Key Responsibilities:
* Assess, diagnose, and develop effective clinical management plans.
* Provide face-to-face or telephone consultations for undifferentiated patient presentations.
* Manage acute and chronic conditions with confidence and compassion.
* Offer patient education and support, promoting self-management and healthy choices.
* Contribute to the delivery of practice policies and procedures.

Person Specification
Experience
* Experience of working in primary care (not necessarily as an ACP).
Qualifications
* Registered ACP with relevant qualifications (MSc or equivalent in Advanced Clinical Practice)
Skills
* Ability to assess, diagnose, and manage a range of conditions autonomously
* Strong communication skills and a collaborative approach to patient care.
