Specialty Doctor in Care of the Elderly / Frailty

Applications are invited for a Specialty Doctor in the Care of the Elderly / Frailty service. The successful candidate will be based at Leighton Hospital in Crewe, Cheshire.
The successful candidate will work with the Consultant Care of the Elderly and Frailty Physicians and will be expected to liaise with other disciplines within the Trust.
The successful applicant must have full registration with the GMC and continue to hold a Licence to Practise.
Main duties of the job
The successful candidate will be based on our Care of the Elderly ward carrying out comprehensive geriatric assessments of our frail elderly cohort of patients which will be supported by members of the multidisciplinary team.

Person Specification
Qualifications
* MBBS or equivalent medical qualification
Fitness to Practise
* Full GMC Registration with a Licence to Practise
* Applicant's knowledge is up to date and fit to practise safely
* Evidence of achievement of Foundation competencies (or equivalent) by time of appointment in line with GMC standards in Good Medical Practice
* Evidence of achievement of ST1 & ST2 competences (or equivalent) in a relevant specialty
Experience
* Acute care safe: up-to-date ALS. Out of hours experience relevant to the job.
* Clinical judgement: experience in making clinical decisions and managing risk. Knows when to seek help, able to prioritise clinical need
* Relevant specialty clinical knowledge: capacity to apply sound clinical knowledge relevant to the job, specialty knowledge exam.
* Proven ability to work effectively in different clinical settings required in the job.
Skills
* Comprehensive Geriatric Assessment: understands the components of a comprehensive geriatric assessment and is confident in initiating the assessment in a frail person.
* Advanced Care Planning: has the ability to recognise patients who would benefit from advanced care planning and end of life care discussions and is able to carry out these discussions with patient and families.
* Supervision: ability to supervise junior team members and assist in conducting audits/QIPs.
Communication Skills
* Effective communication skills: demonstrates clarity in written/spoken communication and capacity to adapt language as appropriate to the situation
* Empathy and sensitivity: capacity to listen and take in others' perspectives.
* Works in partnership with patients: always considers patients' preferences when discussing treatment options.

* Team working: demonstrated experience working in a team, values the input of other professionals in the team.
