Job summary As part of the transformation of young people's mental health care under the NHS Long Term Plan, an opportunity has arisen for an Advanced Social Work Practitioner/AMHP to work across the Acute CAMHS pathway Services (CAMHS Crisis & Intensive Home Treatment (CCIHT), Inspire CAMHS In-patient and Contact Point) with a lead Social Worker/ AMPH role within CCIHT. The service promotes a partnership model of integrated working with children and young people by mental health and social care services. This post will play a vital role in enhancing the team's understanding of the social model in precipitating mental health crises, and promoting creative, alternative responses to crisis and support in crisis. The integrated model will improve recognition of shared professional values with an emphasis on skills development, shared delivery of care and greater multi-agency working. The post will work in a restorative, recovery-focused way to support children and young people experiencing crises and challenges around their mental health. The role will build relationships with young people and their families to get to the core of the causal/exacerbating factors in a crisis, making this a key part of the assessment process. The provision of direct social work interventions will support children and young people to achieve better outcomes. The post holder will be required to take a leadership role in relation to delivery of social work and social care interventions. Main duties of the job The post will be an integral part of the multi-disciplinary team, undertaking assessments, support planning and direct work with children and young people and their families. They will also be a source of and lead on the social model and provide legal expertise and guidance as an experienced social worker and AMHP. As a qualified AMHP the post holder will maintain a strong link with the Hull AMHP Service and will be required to contribute to the Hull AMHP rota. As an AMHP and Advanced Social Work Practitioner; to liaise with key partner agencies to develop and maintain multi-disciplinary and inter-agency working. To provide a high-quality comprehensive S13. considerations for people coming into contact with the service, involving as appropriate the views and input from Nearest Relatives, Carers, Service Providers and Partner Agencies (HTFT and External).
