Job summary We are seeking an experienced and dedicated Advanced Practitioner to provide expert nursing care for patients diagnosed with heart failure with preserved ejection fraction part of Managing Heart Failure at Home on a 6 months fixed term contract. This exciting project is sponsored by NHS England, King's College Hospital NHS Foundation Trust and South London ICB. In this role, you will manage a caseload of patients with heart failure with preserved ejection fraction, ensuring they and their families receive comprehensive clinical and social support. You will play a key role in developing evidence-based nursing practices aligned with current research and guidelines.
