Job Title: Adult Social Worker – Assessment and Intake Team
Location: Southwark
Rate: £32 per hour
We have an exciting opportunity for a dedicated Adult Social Worker to join the Assessment and Intake Team for our client based in Southwark.
What will your responsibilities be?
You will be part of a vibrant and fast-paced team based in the community, providing an initial assessment function for the Older Persons and Physical Disability Service. Your role will involve completing Care Act Assessments, putting in place long-term packages of care, undertaking carers assessments, conducting urgent assessments for care and respite, and assessing for placement in residential and nursing home settings.

Qualifications and Experiences:
To be successful in this role, you must have Social Work England registration, be eligible to work in the UK, hold a qualification in Social Work at degree level, and have post-qualified experience.
