An exciting opportunity has arisen for an enthusiastic and motivated Community Support worker to join the Integrated Community Nursing Care Team.
The post holders will provide and maintain high quality, effective, person-centred health care to people in their own homes, liaising closely with the District Nurse Team Leader and their Deputy.
Part Time hours are available.
Main duties of the job
1. To assist in providing and maintaining high quality effective health care as part of an Integrated Nursing Team.
2. To implement packages of care and re-enablement programmes for patients as required to maintain them in their home environment, working under guidance and supervision.
3. To provide and maintain high quality effective health care, liaising closely with District Nurse Team Leader and their deputy, and providing clinical support to health care support workers.
4. To support case management of complex patients within the wider team.
5. To support Multi Disciplinary Team working in collaboration with key partners.
6. To ensure service delivery is in line with the Core Offer and the Standard Operating Procedures of the Community Nursing Service.

Person Specification
Qualifications
* Long Term Condition Monitoring
* Venepuncture
* Catheter Care (or willingness to undertake/work towards within 12 months)
Experience
* Experience of working in a caring environment.
* Experience of working in a community setting.
* Experience of working with older adults.
* Experience of MDT working.
Knowledge
* Knowledge of working in Community Nursing.
* Awareness of NHS issues.
Skills and Abilities
* Good communication/interpersonal skills - including written and oral.
* Ability to work on own initiative and unsupervised.
* Ability to work as part of a team.
Attributes
* Due to the requirement to undertake visits and transport non-portable equipment, the post holder is required to be either a car driver with a valid driving licence, or have access to an alternative means of transport.
* Motivation and willingness to learn new skills.
* Flexibility.
* Ability to contribute to shift patterns.
* Willingness to move base to support service requirements.

Salary: £24,071 to £25,674 a year pro rata for part time staff.
