An opportunity has arisen to join Wolds Community Wellbeing team as a care co-ordinator.
Under the current legislation, primarily the Care Act 2014, Mental Capacity Act 2005, and the Human Rights Act, you will be completing strength-based assessments, care and support planning, and undertaking reviews with service users in their own home.
A key aspect of the role is the promotion of independence, wellbeing, and choice. There is an expectation that you will have creative conversations with the people to focus on goal setting and outcomes.
The role involves working with people over the age of 18, their families, and carers with a key focus on supporting people to remain independent within their community.
We are looking for an individual with experience of working with adults, well-developed communication skills, and the confidence to work collaboratively with a range of professionals to ensure the eligible needs of service users and their carers are met.
Regular supervisions and support will be provided, as well as an induction into the role.
The role will allow you the opportunity to develop a range of skills and knowledge with a focus on continuous improvement in relation to the quality of work in a team that is supportive, which allows social care practice to thrive.
Currently, the team operates Monday to Friday, but this may be extended to cover a 7-day service in the future.
The ability to travel across a wide geographical area is necessary for which a mileage allowance is payable.
