Typical day as a Support Coordinator
1. Signing new customers up to tenancies in their new homes.
2. Supporting customers to manage a tenancy, helping them to prepare for a future of independent living.
3. Carrying out risk assessments, support planning, goal setting, and regular reviews.
4. Visiting customers to carry out support sessions through the Home Achievement Program.
5. Having meaningful conversations and collaboration with colleagues.

You bring
1. Passion to support our customers to live their best life, working collaboratively with an eye for detail.
2. Experience of creating person-centred support plans and supporting colleagues in working to those plans.
3. Experience of coordinating and assessing customer referrals.
4. The ability to work on your own initiative, remain calm under pressure, and have a resilient approach.
5. A vehicle insured for business purposes; we’ll pay your mileage!

Job details
1. Monday to Friday 9-5 Hybrid - community and office based.
2. Flexibility is important, and you may need to adapt your start or finishing times on occasion to meet the needs of our customers.
3. Able to use technology for creating and updating support plans, completing online learning, and collaborating with colleagues.
4. You’ll need an Enhanced DBS check done, and we pay for that.
