The Community Therapies Team is multidisciplinary in nature and provides patients with access to friendly individualised holistic assessment and treatment within their own homes, and is fully integrated into the wider Community Teams based in Burton-on-Trent.
To support the development of this team we are looking for an enthusiastic and passionate Band 4 Assistant Practitioner with skills working with patients with both Occupational Therapy and Physiotherapy needs, to join us to make a difference to people's lives within our community in East Staffordshire.
You will work alongside Physiotherapists, Occupational Therapists and support workers to provide home-based assessments and the implementation of treatment plans for patients in the community who are usually recovering from illness or injury affecting their independence.
This team has a 100% supervision recording and works seamlessly with Rapid Response, Community Neuro and Falls Teams and D2A services, and has well-established links with the Acute Sector and Primary Care.

Person Specification
Qualifications & Training
* Undergraduate degree, or evidence of equivalent study.
Experience
* Experience as a Physiotherapy or Occupational Therapy assistant.
* Experience working in the NHS
* Experience in working with people to develop and support their independence
