Job summary We are seeking an enthusiastic, self-motivated and well organised Home Visiting Practitioner to work 22.5 hours per week over 3 days within our Primary Care Network Home Visiting Service. You will be a dedicated professional who shares our values and passion about delivering high quality, joined-up clinical management of patients in their own homes. Main duties of the job You will have the clinical expertise and ability to autonomously assess and treat patients with a range of conditions and be comfortable acting autonomously and seeing patients without prior contact with their GP. You will have experience in primary care working and be confident in formulating rapid and accurate treatment plans.
