Role Summary
The Urgent Community Response Team Manager (UCRTM) will be responsible for the leadership and delivery of Urgent Community Response Service across the cluster through the operational management of NHS employed Occupational Therapists, Physiotherapists, Registered Nurses and Urgent Community Response Support Workers.
Using a strength-based approach, the post holder will be part of a wider integrated health and social care team who are responsible for the assessment, implementation and evaluation of service delivery to promote high-quality therapeutic intervention and patient care.

Are you a confident clinician (Registered Nurse, Occupational Therapist or Physiotherapist) who is passionate about community services, supporting people to return home and preventing hospital admissions?
