An exciting opportunity for a Clinical Practice Facilitator to support our clinical patient facing staff in practice has become available.
The purpose of this role is to ensure theoretical learning is consolidated in practice by competencies sign offs and support.
DCHS are seeking a highly motivated individual who will be enthusiastic about providing an intensive support programme for new and existing staff.
You will be an experienced clinician and have excellent communication, IT and clinical skills with the ability to work well within a team and independently.
You will have a keen interest for supporting and developing others, using up to date evidence based practice, sharing knowledge and skills to consolidate the theoretical learning into practice.
Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.
We employ around 4,200 substantive staff, caring for patients in 8 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people's own homes and via virtual consultations.
