This dynamic, forward-thinking, and friendly PCN is looking for an Enhanced Nurse Practitioner to work within a surgery setting alongside GPs, the Nurse Manager, Partners, PCN staff and other members of the wider health team. You will be contributing to the achievement of QOF, the achievement of Enhanced Services, delivering objectives set by the PCN, Practice and governing bodies and audits and protocols. Ensure patients have assessments made of their healthcare needs & that individual programmes of care are planned, implemented & reviewed. Facilitate patient referrals.
