To be responsible for implementing an effective medicine management system within the practice, including cost-effective prescribing, working as an independent prescriber, providing advice and support for both patients and colleagues. In addition, the post-holder will also support the multi-disciplinary team, in line with the strategic objectives of the practice.
Main duties of the job
1. To provide medication review services with patients in the practice and during domiciliary visits to the local nursing home.
2. Encourage cost-effective prescribing within the practice.
3. Review all safety alerts ensuring the required actions are taken.
4. To reconcile medicines following patients' discharge from hospital or clinic appointments.
5. Review medications for newly registered patients.
6. Implement and embed a robust repeat prescribing system.
7. To provide subject matter expertise on medication monitoring, implementing and embedding a system.
8. Carry out targeted medication reviews for high-risk patients.
9. To re-authorise medication for repeat prescribing within the scope of practice.
10. Identify patients suitable for repeat prescribing, liaising with the relevant clinicians as required.
11. Review and implement safe prescribing systems for vulnerable patients, improving compliance.
12. Assist with QOF targets.
13. Partake in audit as directed by the audit lead.
14. Review the latest guidance ensuring the practice conforms to NICE, CQC, etc.
15. Act as the practice point of contact for all medicines related matters, establishing positive working relationships.
16. To line manage the dispensary staff in accordance with current practice policy.

Job responsibilities
Sutton Road Surgery is seeking a motivated and compassionate Pharmacist who has EMIS training to join our multidisciplinary team. In this role, you will be responsible for managing medication therapies, offering clinical advice to patients and healthcare professionals, and promoting the safe and effective use of medicines across the practice. You will be a key member of our healthcare team, working closely with GPs, nurses, and other healthcare providers to deliver excellent, patient-centered care.
* Medication Reviews: Conduct regular medication reviews for patients with chronic conditions (e.g., diabetes, hypertension, asthma) to ensure optimal drug therapy.
* Patient Counseling: Provide advice on the proper use of prescribed medications and over-the-counter products, helping patients understand their treatment regimens, manage side effects, and improve adherence.
* Medicines Optimization: Work with the clinical team to review and optimise medications for patients, ensuring that treatments are effective, safe, and appropriate to the individual's needs.
* Clinical Advice: Offer expert advice on drug interactions, potential side effects, and alternative therapies to support clinical decision-making.
* Health Promotion: Contribute to patient education and health promotion initiatives such as smoking cessation programs, weight management, and prevention of long-term conditions.
* Medication Audits: Conduct regular audits to ensure that the practice is compliant with national guidelines and local regulations regarding medication storage, prescription practices, and patient safety.
* Regulatory Compliance: Ensure compliance with all pharmaceutical laws and regulations, including those related to controlled substances, medication safety, and record-keeping.
* Prescription Management: Monitor prescription patterns and make suggestions for improvements in prescribing practices to enhance patient safety and cost-effectiveness.
* Ongoing Learning: Engage in continuing professional development activities to keep up to date with the latest developments in pharmacy practice, new medications, and clinical guidelines.
* Training: Provide training and guidance to other team members regarding pharmaceutical matters, ensuring everyone is informed about the safe use of medicine.

* A valid MPharm degree and registration with the General Pharmaceutical Council (GPhC).
