Job summary An exciting opportunity has arisen for a Band 6 prison nurse to join the hepatitis CNS team. This post will require someone with experience and background of working in a prison environment (Not as a Student). An interest in hepatitis C will also be an advantage. This post is for 15 hours a week only. The post holder will work closely with the Hepatitis CNS's in delivering patient focused care in HMP Birmingham, Category B prison. They will help run a nurse led service in treating patients with hepatitis C in the prison, help with testing in the prison and link in with the Hepatitis C Trust and other Health and Justice partners. You will be offered support and supervision by the senior CNSs and will undertake a competence based training programme. The ideal candidate will be able to demonstrate a sound clinical knowledge of working within the prison setting and is essential for this post. Within this exciting role there are plenty of opportunities to develop and the successful candidate will be able to explore opportunities to enhance their knowledge and skills. The post holder will : Provide specialist nursing advice to patients referred for specialist intervention, care and support within the hepatitis services, being a point of contact for assessment, information and advice for the patient, family and carers. Be a point of contact and liaison with the prison Blood borne virus (BBV) link /lead nurse. To liaise closely with the Hepatitis C prison peers and to participate in the "Follow Me Project" to track the patient after release to ensure we get the outcome SVR 12 test Continuously assess communication, educational and information needs for patients devise plans to ensure needs are met, ensure instructions/information are understood. Report and raise concerns about Safeguarding with HMP prison staff and accessing advice and support at the point of need. Establish and maintain robust communications process within and external to your team. Ensure effective handover of patients between health care professionals using a range of communication aids with focus on offender release into the community.
