Main Duties and Responsibilities Lead the design and delivery of specific work streams Ensure opportunities for greater alignment and integration of priorities are built into the design and planning of the workstreams and have been informed through effective engagement with relevant stakeholder within our Partnerships Work collaboratively to ensure alignment between place based and WY System wide priorities To be an active member in cross-Directorate discussions on integrated care and transformation, including representing the function at other ICB working groups, identifying, and progressing opportunities to further join-up initiatives or build capacity To develop communications materials To work alongside health and care colleagues to develop, test and embed new and innovative ways of working together with a specific focus on tackling health inequalities and system challenges. Work with key LHCP stakeholders and partners at local level to develop the priority programmes of work to deliver the Leeds strategy and agreed WY approach Lead and manage the development of our place plan through co-production across the partnership, ensuring alignment to Leeds Health and Wellbeing Strategy, regional and National strategy and plans Undertake assessment of needs with key partners and stakeholders in relation to work streams/projects To oversee the effective deployment and utilisation of funding Work with colleagues in health and care to support a shift to greater investment in prevention and community-based solutions with a focus on those population groups least likely to access mainstream health services or experiencing health inequalities. Provide programme management and co-ordination of a broad range of complex activities within multiple projects across a range of partners and stakeholders Monitor progress on delivery of workstreams, ensuring reporting and appropriate escalation of key risks and issues To support the development, use and interpretation of datasets and business intelligence reports and dashboards, both nationally and within the ICB, Manage own workload and make informed decisions independently on behalf of projects with appropriate and timely escalation of relevant risks and issues to the senior leaders within the directorate where appropriate. Undertake or facilitate research and development activities that may be required to ensure best practice and innovation is embedded Contribute to evaluation plans to demonstrate impact and realisation of benefits. To seek out, interpret and implement national policy initiatives and assess application for local use. Ensure all work builds on community-based assets and that co-production is embedded as a working principle across in all our work. To act as a catalyst for change and innovation with a focus on community-based solutions, prevention and reducing health inequalities. Undertake statistical analysis and presentation of highly complex information, data statistics to inform and make decisions on aspects of work within the function. Manage an agile team to support delivery within the post holders areal of responsibility and wider business unit
