Job summary We are looking for an autonomous, independent and experienced Experienced Advanced Nurse Practitioner to join our multidisciplinary team of clinicians and be part of a growing establishment who are committed to being a flagship organisation and at an scale provider of primary care services in the Midlands. The role is based in the Chesterfield area - 37.5 hours per week. We are a team of 200 people dedicated to our patients, working across 3 sectors covering Chesterfield, Clay Cross and Heanor. You need to be passionate about our patients, with a desire to improve our population health and wellbeing by shifting from treatment to prevention. Main duties of the job Acting within your professional boundaries, you will assess, diagnose, and manage patients with a range of acute, non-acute and chronic medical problems. Managing daily surgeries comprising of face-to-face appointments, telephone appointments, telephone triage and undertaking home visits when appropriate. You will be a first point of contact within the practice for patients presenting with undifferentiated, undiagnosed problems, using your skills and experience to establish a diagnosis and management plan. You will clinical assess and manage patients with chronic and ongoing health problems. You will use your individual skills and experience to make a positive contribution to the organisation and the development of a "one team" approach to provide high quality patient services within and across primary care. This is an essential role working with GPs, other Advanced Nurse Practitioners, Practice Nurses, Pharmacists and Mental Health Practitioners in order to succeed.
