Job summary This is an exciting opportunity for a registered nurse or health care professional to join the expanding lipid team to improve the care and outcomes for people with and at risk of CVD by supporting primary care towards detection and management of hyperlipidaemia (including Familial Hypercholesterolaemia FH). Main duties of the job Work in the community to identify and treat people at risk of and with pre-existing cardiovascular disease. Search primary care databases, contacting individuals for intervention, counselling, treating and following up, referring onwards to the lipid clinic as appropriate. Identify people with possible Familial Hypercholesterolaemia and refer to the lipid clinic Utilize skills of clinical expertise, education and management to ensure a seamless service for patients, carers and staff. Work across divisional, organisational and professional boundaries, encouraging inter-disciplinary and inter-agency working for the benefit of improved patient outcomes. The precise details of service delivery will vary across differing practices and primary care networks (PCNs). Deliver education and participate in audit and research.
