Job Description
As a Social Worker within the Children with Disabilities Team, you will work with children and young people who have complex needs, including physical and developmental disabilities.
Your responsibilities will include assessing their needs, creating individualized care plans, and facilitating access to services that enhance their independence, well-being, and overall quality of life.
This role focuses on delivering child-centered support while collaborating closely with families and multi-agency professionals to achieve the best possible outcomes.
Key responsibilities include:
* Understanding the needs, risks, and strengths of children with disabilities and their families.
* Collaborating with families, health professionals, and education providers to gather holistic information.
* Managing a caseload of children with a range of disabilities, ensuring regular review and adjustment of care plans.
* Identifying and responding to any safeguarding concerns, ensuring that children are protected from harm, abuse, or neglect.
* Advocating on behalf of children and young people to ensure they have a voice in decisions about their care and support.
