Job Purpose Working alongside a social prescribing link worker and care coordinator, taking referrals from GPs and other professionals in the network. The post holder will spend time with the people referred, building trusting relationships, listening carefully to what matters to them and what motivates them, and supporting them to take action on their self-identified goals. Key Responsibilities Work with GPs and the wider clinical team to identify and manage a caseload of patients likely to benefit from health and wellbeing coaching, taking referrals from practices within the PCN for individuals who would benefit from support. Work closely in partnership with the Social Prescribing Link Worker to identify and work alongside people who may need additional support, but are not yet ready to benefit fully from social prescribing. Manage and prioritise a caseload, in accordance with the health and wellbeing needs of the local population through a non-judgemental approach, based on strong communication and negotiation skills. Utilise existing IT and MDT channels to screen patients, aiming to identify those that would benefit most from health coaching. Provide personalised support to individuals, their families, and carers to empower them to manage their own health and wellbeing and live independently. Coaching and motivating patients through multiple sessions to identify their needs, set goals, and achieve their personalised health and care plan objectives. Providing interventions such as self-management education and peer support. Supporting patients to establish and attain goals that are important to them. Encouraging positive risk-taking while ensuring understanding of accountability, thus fostering the proactive prevention of further illnesses. Working in partnership with the social prescribing service to connect patients to community-based activities. Increase patient motivation to self-manage and adopt healthy behaviours. Support patients in shared decision-making conversations. Explore and support patient access to a personal health budget, where appropriate. General Ensure all activities are monitored and evaluated. Attend internal meetings as required, and work in accordance with PCN policies. Participate in clinical governance activity and contribute to the improvement in quality of health outcomes through audit, risk management and the Quality and Outcomes Framework. Connectivity with Other Sectors Develop collaborative relationships with health, social care, and community and voluntary sector providers to holistically support patients. Utilise population health data to tackle local inequalities in health and social care. Confidentiality Maintain confidentiality of information about patients and staff under the Data Protection Act 2018 and the EU General Data Protection Regulations. Special Working Conditions This post requires travel from practice to practice and to other venues. Flexibility in working hours is required.
