The Cardiac Rehabilitation programme delivered by the team has current British Association for Cardiovascular Prevention and Rehabilitation (BACPR) GREEN certification status.
You should have excellent communication skills and ability to work independently as well as part of a multidisciplinary team.
Your vision and leadership will drive you to further develop this service to meet the individual needs of cardiac patients along with their families/carers.

The post holder will be responsible for providing effective clinical leadership supporting a team of nurses and therapists ensuring the Nursing & Midwifery Council (NMC) code of conduct is at the forefront of the team’s nursing care. The position encompasses a clinical role in addition to the management of operational issues and the provision of expert health advice in relation to cardiac rehabilitation. The post holder will be responsible for the management of nursing practice and patient care delivery within the cardiac rehabilitation department, ensuring the smooth running of the service.
The post holder will lead and manage the development of the Cardiac Rehabilitation Service. This involves co-ordination of an inter-disciplinary team to provide support, information and guidance to the patient, their partner and /or family, and other health professionals. The post holder will also implement and evaluate rehabilitation programmes which are integrated as part of the patient’s ongoing care in both the hospital and community settings.
The post holder will work closely with the multi-disciplinary team in the provision of a high standard of individualised patient care, thus improving the patient experience. The post will incorporate the provision of care to patients, their families and carers in accordance with the philosophy and standards of care within the clinical area.

• Act as a clinical expert in their area of practice. To work clinically as part of the Cardiac Rehabilitation team, to share experience, knowledge and expertise.
• Using specialist skills and knowledge identify and assess patient’s suitability for Cardiac Rehabilitation and refer to other professionals as appropriate.
• Manage own caseload of inpatients and outpatients.
• Work clinically as an autonomous practitioner in virtual and face-to-face Cardiac Rehabilitation clinics. • Order appropriate pathology testing, ECGs and echocardiography.
• Demonstrate evidence based physical assessment and clinical evaluation skills.
• Work as an Independent Nurse Prescriber within the Scope of Practice for the role to reduce the impact of symptoms through achieving effective medical therapy.
• Offer specialist information, advice and support to patients/carers from the time of diagnosis, throughout treatment and follow up.
• Analyse and respond to complex clinical situations utilising specialist knowledge to Assess, plan, deliver and evaluate individual programmes of care for each patient that meet the British Association for Cardiovascular Prevention and Rehabilitation (BACPR) standards and Core components.
• Signposting and referring where appropriate patients to other health professionals or supportive services e.g., smoking cessation, dietetic team, psychology services.
• Accurate and legible documentation should be completed to record cardiac assessments/interventions.
• Providing good written communication to the patients GP and patient regarding individual assessments and behaviour changes/goals agreed.
