Job summary An exciting opportunity has arisen for an experienced Physiotherapy support worker to join the community rehab team based at St Austell Hospital. We are looking for someone who already has experience of working as a therapy support worker and who is ready for the challenge of working more autonomously in the community or someone with previous community therapy support work. The role will involve working with various physical presentations including frailty, neurological conditions and orthopaedic and stroke This post is community based and so a car driver with access to a car is essential. The post holder will work closely with both Physiotherapy and Occupational Therapy staff and be part of the developing PCN working. This post is for 20 hours a week with working days Monday to Friday. We welcome contact prior to application/interview if you wish to know more about the post. Main duties of the job Under the guidance of the Physiotherapist you will be responsible for the assessment, treatment and discharge planning of patients within your care. You will need to assist in the interpretation of information relevant to the patient's condition/progress, identify problems and patient needs, setting realistic goals. You will need to have excellent organisational skills combined with experience of working in older person's services. You will need a valid UK driving licence and the ability to travel to different locations to facilitate hospital discharges. We are a supportive team, including OTs / PTs and support workers with excellent MDT working with other disciplines.
