Job Overview
We are looking for Band 5 nurses to work within our district nursing teams based in Sefton Place, to cover our 8am-8pm 7 days a week model. The service is undergoing significant developments as part of our plan to deliver integrated community care for patients on a neighbourhood footprint across Sefton.
Working to the Mersey Care values, you will have excellent empathy, communication skills and a person-centred approach. You will be keen to continue learning and develop the competencies needed to give evidence-based care to your patients in their own home. Clinical decision-making skills and a problem-solving approach are essential components of this role. You will be a good team player who can also work autonomously on your own, escalating and reporting back appropriately.
Main Duties of the Job
The post holder is responsible for the delivery of clinical care to a defined population. Supported by senior practitioners, they will deliver care within the boundaries of their role. They will work effectively and collaboratively within the team to meet the needs of patients, support the delivery of policy and procedures and provide clinical leadership.
Detailed Job Description and Main Responsibilities
1. Assess, plan, develop, implement and evaluate programmes to promote and prevent adverse effects on health and well-being.
2. Implement and evaluate individual treatment plans for patients.
3. Identify and manage treatment plans for patients at risk.
4. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations.
5. Provide information and advice on prescribed or over-the-counter medication.
6. Recognise, assess and refer patients presenting with mental health needs.
7. Promote and deliver evidence-based care.
8. Assist senior practitioners as required.
9. Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment.
10. Communicate effectively with patients and carers, recognising the need for alternative methods of communication.
11. Act as an advocate when representing patients and colleagues' viewpoints.
12. Produce accurate, contemporaneous and complete records of patient consultation.
13. Deliver care according to national and local guidelines.
14. Participate in the maintenance of quality governance systems and processes across the organisation.

Qualifications
* Registered Practitioner
* Mentor/teaching qualification or equivalent experience
Knowledge/Experience
* Evidence of clinical experience in relevant field.
* Clinical supervision experience
Skills
* Excellent communication, organisational and interpersonal skills
* IT Literate
* Access to a full driving license and ability to travel across boundaries
