Registered Clinician, or experience of working within health or social care discharge planning role Ongoing evidence of continuing professional development Experience of working across organisational boundaries within health and social care Knowledge and understanding of current legislation and best practice in relation to key areas including Information Governance, Care Act, Mental Capacity Act. Up to date understanding of National Hospital Discharge policies and guidance Ability to manage, and adapt to change within multiple teams Ability to contribute to, monitor and implement changes and improvements to services Knowledge of safeguarding policies and procedures Ability to interpret clinical information to support risk assessments when planning discharges Excellent communication skills, both verbal and written, and the ability to influence others in clinical decision making towards discharge Able to change priority, often at short notice Creative approach to system pressures Sound conversational skills allowing for difficult conversations with both patients, families, and staff Ability to remain calm and consider wider picture in particularly stressful situations.
