Duties and Responsibilities\n\nTo arrange and monitor appropriate services in line with individual care plans.\nTo provide advice and practical help.\nTo liaise with voluntary and statutory agencies.\nTo complete annual care plan reviews and or reassessments.\nTo participate in team meetings and duty rotas.\nTo undertake these duties in line with Council Policies and Procedures.\nTo undertake training and self-development and to contribute to this process in others. \n\nCorporate Requirements\n\nTo take reasonable care for the health and safety of yourself and other persons who may be affected by your acts or omissions and to comply with all health and safety legislation as appropriate.\n\nAs a term of your employment you may be required to undertake such other duties and/or times of work as may reasonably be required of you, commensurate with your grade or general level of responsibility within the organisation.\n\nAlthough you will be provided with a base, you will be required to work from various locations in accordance with the needs of the role.\n\nKnowledge and Experience\n\nExperience of working with older people and adults with a physical disability through direct contact in a working environment. \nAbility to work independently and as part of a team.\nAbility to make relationships with se5rvice users and carers.\nAdministrative ability\nGood communication skills\nOrganisational skills\nUnderstanding of the needs of elderly and or disabled people.\nWillingness to undertake appropriate training courses.\nEmpowerment of disabled and or elderly people and their carers.\nUnderstanding of the needs of elderly and or disabled people.\nWillingness to undertake appropriate training courses.\nEmpowerment of disabled and or elderly people and their carers.
\n\nDesirable\n\nFull Valid Driving Licence\nEducated to GCSE standard or equivalent\nNVQ/GNVQ Level 3 or 4 in Social Care\nExperience of working with staff from a wide variety of agencies.\nExperience of working within a hospital setting
