Job summary The post holder will work as a Community Staff Nurse in the Urgent Community Response team. You will deliver responsive care to patients in their own environment. You will need to be able to prioritise urgent patient care dependant on the patients need and clinical condition. This will help to support patients to be cared for in their own environment and prevent hospital admission. Main duties of the job The post holder will deliver high standards of patient care for patients in the community by completing clinical assessments and delivering care interventions to patients in their own environmentsThe post holder will complete Clinical Triage utilising the triage templates on System One, this in conjunction with effective communication and listening skills will support the patient to be referred onto the correct community pathway.The post holder will work closely with Multi-Disciplinary Teams to deliver the appropriate care to ensure that clinical patient outcomes are achieved.The post holder will empower other team members to perform their roles effectively supporting them with expert clinical advice.The post holder will be required to perform a number of clinical skills in the community environment. These will include administering IV antibiotics, assessing and redressing complex wounds and providing appropriate support and care interventions for patients that have a palliative care diagnosis.
