Job responsibilities
Assessingand treating patients in their own place of residence and those attendingsurgery.
Triageand treat patients, making any necessary referrals to other members of theprimary health care team and MDT and/or organise admission to hospital ifrequired.
Ensureclinical practice is safe and effective and remains within boundaries ofcompetence and acknowledges practitioners own limitations.
Advisepatients on maintaining health and wellbeing and refer to other team members asneed arises.
Communicateat all levels within the multidisciplinary team ensuring effective service isdelivered.
Maintainaccurate, contemporaneous healthcare records appropriate to the consultation.
Ensureevidence-based care is delivered at the highest standards ensuring delivery ofhigh-quality patient care.
Workwithin local policies and procedures.

Essential
1. HCPC Registered.
2. QOF knowledge Make every contact count.
3. Knowledge of primary and community healthcare.
4. Excellent communication skills.
5. Good organisational and IT skills.
6. Ability to manage and prioritise workload.
7. UK Driving License & Business Insurance must be held for Home Visiting Service.
8. Clear understanding of professional responsibilities and accountability.
Desirable
9. Experience of SystmOne / EMIS clinical system.
10. Experience working in a GP practice / Primary Care.
