Job summary As a Band 6 Community Practitioner you will coordinate mental health care and treatment for patients within our locality area with cognitive and functionally frail conditions focusing on care in the community. You will provide thorough assessment and treatment plans within NICE guidelines to a caseload of patients across Sunderland. You will be able to work autonomously and as part of a team. Our Older Adults Pathway is a multi-disciplinary team with other functions available which can support with complex patient needs such as our Day Hospital, Step Up Service, Behaviour Support Service, Physical health monitoring and Care home liaison team. It will be expected that you will possess excellent communication, interpersonal skills and have an ability to manage your time effectively. You will work in a highly supported environment and will be provided with training opportunities to develop your knowledge and skills.
