Job Summary
We are currently seeking a RN to join the Community Nursing service. The role involves providing holistic nursing care to adult patients in their own home or residential care setting. Successful applicants will be required to hold a first level nursing registration with the NMC; additional training specific to the role can be provided.
Main duties of the job
The Community nursing team provides both planned and reactive care including Urgent Community Response.
About Us
We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community-based settings and providing care in patients' homes.

Job Description
Job responsibilities:
The successful candidate will provide a variety of nursing care to patients; including wound care, administration of medication, palliative care, management of bowel and catheter care. The role involves assessment of patient care needs, care planning, delivery of care and review. The successful candidate will be required to work independently as part of the wider Nursing team reporting back to senior staff on duty.

Special Requirements Essential
* Able to travel across Trust sites to meet the requirements of the post.
Qualifications Essential
* 1st Level Registration with the NMC (Adult).
Desirable
* Knowledge of issues in community nursing.
* Knowledge of Intermediate Care.
* Knowledge of key policy drivers for health and social care.
Experience Desirable
* Experience of audit/research.
* Experience of clinical supervision.
* Evidence of recent ongoing nursing development.
Statutory Registration Essential
* 1st Level Registration with the NMC (Adult).
Special Skills & Knowledge Essential
* Good communication and interpersonal skills.
* Evidence of organisational skills.
* Evidence of team working.
Desirable
* Community specific skill set.
