Location: SouthEast
Pay rate: £36,648 - £38,186 per Annum
Are you a Qualified Social Worker looking for a new opportunity within the Hospital Discharge Team? We are working with a local authority in the South East seeking experienced social workers for their Hospital Discharge Team.
In this role, you will work in accordance with social care legislation and local government requirements, supporting the delivery of person-centred, safe, and effective social care services. You will make assessments and decisions about appropriate interventions, leveraging your professional knowledge and experience. Building relationships with service users and agencies will be key to ensuring services meet needs.
You will be committed to positive and sustainable outcomes, striving to provide the best results for children and supporting children and families to thrive. Our priority is to protect the vulnerable and operate efficiently.
Ideal Candidate:
* Qualified Social Worker registered with Social Work England
* Experience working with children and families, including vulnerable groups
* Creative, flexible, and forward-thinking approach
* Knowledge of Child Protection legislation and issues
* Dedicated to continuous improvement for children’s outcomes
* Excellent mediation and negotiation skills
* Ability to build meaningful relationships with children and families
* High-quality service commitment
* Experience in multi-disciplinary, interagency work
* 4+ years qualified
Responsibilities:
* Assess risks, protection, and social care needs
* Manage caseloads
* Use specialist communication skills for optimal outcomes
* Develop individual care plans respecting diversity and dignity
* Support and advise unqualified staff and students
