A new and exciting opportunity has arisen for a part-time pharmacist or part-time registered nurse to join our enthusiastic and specialist Anticoagulation team based at Wexham Park Hospital. As well as being responsible for providing a pharmacist and nurse led service, the successful candidate will be expected to complement the existing team and contribute significantly to the ongoing development within our field.
We require someone who is ready for a change and to assist us in maintaining our excellent Anticoagulation Service. The role involves providing Warfarin dosing, running clinics and management of DOAC / LMWH. You will work independently within the Anticoagulation Team, be self-motivated and require excellent communication, IT and keyboard skills.
Liaison and communication with other professional groups, agencies and the user group, both within the hospital and in the community, will be an essential component of the role.

The post holder will be part of a specialist Anticoagulant team based at Wexham Park Hospital, but covering all areas across the Trust and therefore expected to travel between sites.
* As part of the team, the post holder will provide a comprehensive anticoagulant service to inpatients and outpatients and support appropriate prescribing.
* The post holder will undertake assessment and education of new patients and provide clinical advice and support to patients and other healthcare professionals.
* The post holder will work within the multidisciplinary team of scientific, nursing, pharmaceutical and medical staff.
* The post holder will provide expertise to all professionals in both the acute and community setting.
* The post holder will be expected to act as a role model at all times.
